Care Plan Assistant
I created this project to help me with my job as an MDS coordinator. The purpose was to increase my efficiency at work by saving time on care plans. If you are not familiar with a nursing care plan, it is made up of three parts: a Focus, Goal, and Interventions. Part of my job was to write complete care plans from a list of diagnoses and medications, and doing this by hand was very time consuming. I would have to individually sort each item into a category, then enter them manually one by one into a care plan, which would take a while with lists of 40+ diagnoses and medications (which was very often working with the geriatric population).
I made this with the idea that I could have the diagnoses and medications pulled up on the side of my monitor, and very quickly copy them over and click their category, and upon finishing, I wanted the care plan to be formatted and organized for me. This took time and research to complete, and I had to overcome roadblocks along the way, but I have used this for almost a year now and it has helped me complete care plans at an incredibly quicker rate than before. I use this to form the base line care plan, then I add in patient specific details after.
I have modified the template slightly so as to not use all of my facility’s documentation, but I have replaced them with other real life nursing interventions.
